How can I get speech recognition on my PC?

I need to assist someone who is injured and needs to do speech recognition computing. I understand Vista comes with speech recognition nowadays but the person is using Windows XP. The person will mainly use the speech recognition for Word processing and perhaps for commands to open files/folder, IE, etc. How does this work, do we buy a separate software and set it up? Any suggestions for a good noise free microphone? Please share your knowledge and experience. Thank you.
